Rule 37.96.311 - STAFF RECORDS(1) If a licensed or registered child care provider employs staff, the provider must maintain the following records for each staff member:(a) contact information; and(b) acknowledgment of, and agreement to follow, program policies for: (i) identifying and preventing shaken baby syndrome and abusive head trauma;(ii) mandatory reporting; and(iii) safe sleep, if the program provides care for children under two years old.
