Rule 24.29.635 - RENEWAL
(1) A self-insured employer or employer group must renew each year. The renewal application must be submitted to the department 60 days before the renewal date.
(2) A renewal applicant must:
(a) submit all required documents; and
(b) be in compliance with laws governing plan No. 1 employers and groups.
(3) A self-insured employer or employer group not renewing shall elect to be bound by plan No. 2 or 3 on the effective date of the termination of permission to self-insure.
