Rule 30-2101-2.5 - Outdated LicenseA. Any person who has been licensed in this state, and whose license has expired for a period of three (3) years must make application for approval for written and practical examination. B. The application for examination must be submitted on a form prescribed by the Board and must be accompanied by: 1. Two recent passport photographs of the applicant, taken prior to application. 2.
