Application for a permit shall be submitted on a regular form furnished by the Bureau in time to be approved ten (10) days prior to regular scheduled examinations. No permit application shall be accepted unless the applicant furnishes written proof that he is a bona fide employee of a person holding a license in one or more of the categories listed under subsection 104.04 of this chapter and holds a registered technician identification card.
