Once a family is approved for child care subsidy a child care certificate is issued to both the parent and the provider. In addition to other details, the certificate contains the following:
1) The name of the child approved for care.2) The rate of subsidy reimbursement for which the child was approved.3) Whether the child was approved for full-time care, part-time care, or both; including the days of care requested by the parent.4) The assigned co-payment rate to be billed on behalf of this child.5) The name and ID number of the approved provider location designated for care. Care shall only occur at the location associated with both the provider name and ID number indicated on the certificate. When multiple care locations are owned, care is only allowed at the single location indicated on the certificate. The child care certificate details the conditions under which care is allowed to occur. Any deviation from what was approved will be considered fraudulent and subject to penalties, fines, recoupment, and possible prosecution when warranted.
