Rule 15-5-85-6.3.4

Standardized report forms shall be developed and distributed or made available online in a downloadable format to all medical professional organizations, licensed physicians, hospitals, emergency rooms, and abortion facilities, Department of Health clinics, and ambulatory surgical centers operating in the state.
