Any reports or complaints to the Establishment concerning a tattoo and/or piercing performed at the establishment, whether such complaint be delivered in person, by telephone or other electronic means, shall be recorded in a log which must include, at a minimum, the name of the complainant, sufficient identifying information to cross reference to the identification attached to the consent form, a summary of the complaint, a summary of any advice provided by the establishment, and change in contact information which is different from the original consent form. Log should be readily available if requested by the inspector.
