Rule 15-19-60-11.3.2 - Provisional Certificate of Registration-Tattoo and Body Piercing

An applicant for a Provisional Certificate of Registration for tattoo and body piercing shall submit to the Department, verified by oath, written evidence in form and substance satisfactory to the Department, that the applicant:

1. Is over the age of eighteen (18) years;

2. Will work under the direct on-site supervision of an individual with a "Regular Certificate of Registration" for a minimum of nine months at a registered establishment; and

3. Has successfully completed the American Red Cross "Preventing Disease Transmission" course or a similar course acceptable to the Department.
