A personnel record shall be kept on each staff member and shall contain the following items, as appropriate:
1. Application for employment2. Written references and a record of verbal references3. Verification of all training and experience, and licensure, certification, registration and/or renewals4. Wage and salary information5. Performance appraisals6. Initial and subsequent health clearances7. Disciplinary and counseling actions9. Employee incident reports10. Record of orientation to the facility, its policies and procedures and the employee's position.11. For each position in the facility, there shall be a written job description that specifies the duties and responsibilities of the position and the minimum level of education, training, and/or related work experience required or needed to fulfill it.15 Miss.
