The facility shall maintain written discharge planning policies and procedures which describe:
1. How the discharge coordinator will function, and his authority and relationships with the facility's staff;2. The time period in which each patient's need for discharge planning is determined (within seven days after admission).3. The maximum time period after which re-evaluation of each patient's discharge plan is made.4. Local resources available to the facility and the patient to assist in developing and implementing individual discharge plan; and e.
