Written policies and procedures governing the intake process shall specify the following: a. the information to be obtained on all applicants or referrals for admission; b. the records to be kept on all applicants; c. the statistical data to be kept on the intake process; and d. the procedures to be followed when an applicant or a referral is found ineligible for admission.
