The written plan for professional services shall describe the following:
1. The population served, including age groups and other relevant characteristics of the patient population;2. The hours and days the facility operates;3. The methods used to carry out initial screening and/or triage;4. The intake or admission process; including how the initial contact is made with the patient and the family or significant others;5. The assessment and evaluation procedures provided by the facility;6. The methods used to deliver services to meet the identified clinical needs of patients served;7. The basic therapeutic programs offered by the facility;8. The treatment planning process and the periodic review of therapy;9. The discharge and post-therapy planning processes;10. The organizational relationships of each of the facility's therapeutic programs, including channels of staff communication, responsibility and authority, as well as, supervisory relationships; and11. The means by which the facility provides, or makes arrangements for the provision of the following: a. Other medical, special assessments and therapeutic services;b. Patient education services, whether provided from within or outside the facility;c. Emergency services and crisis intervention; and d. Discharge and aftercare, including post-therapy planning and follow-up evaluation.15 Miss. Code. R. 16-1-40.12.3
