Rule 15-11-55-3.2.7 - Renewal of License1. The licensing agency shall issue licenses that may be renewed annually. The licensing agency shall email a renewal notice to the address of the operator registered with the licensing agency. The operator shall:
a. Complete the renewal application.b. Submit all required documents for approval by the licensing agency.c. Submit the renewal fee.d. File the above with the licensing agency at least 30 days prior to the expiration date on the license. Renewal applications submitted online less than 30 days prior to the expiration date of the license shall be assessed a $25.00 late fee. Application and fees are to be paid online at the Mississippi State Department of Health website (healthyms.com).2. An operator who does not file the renewal application prior to the date that the license expires will be deemed to have allowed the license to lapse. In its discretion, the license may be reinstated by the licensing agency, by payment of both the renewal and the reinstatement fee, provided the application for reinstatement is made within one month of the expiration date of the license.3. After the one-month reinstatement period, an application for an initial license must be submitted. All licensure requirements in effect at the time the new initial application is filed shall be met.15 Miss. Code. R. 11-55-3.2.7
