Section R. 325.11504 - Annual reportsRule 1504.
(1) A type I public water supply which does not submit a monthly operation report shall submit an annual report on a form provided by the department. The department may require certain type II public water supplies to submit annual reports. The report shall include a summary of water pumpage and water use.(2) The type I public water supply shall submit the annual report to the department on or before March 31 following the year covered by the report.Mich.
