Section 590-280-3 - Prescriber Data Set Filing Description

Each prescriber seeking confidentiality protection must submit to the MHDO, at the URL address as indicated in Section 2(A), information which corresponds to each of the following data elements:

A. Drug Enforcement Administration (DEA) Number;

B. Maine Professional License or Certificate Number;

C. National Provider Identification (NPI) Number;

D. First Name;

E. Middle Name;

F. Last Name;

G. Generational Indicator/Suffix;

H. Degree or Credential;

I. Professional Specialty;

J. Practice Name;

K. Practice Physical Address; and

1. City/Town;

2. State; and

3. Zip Code

L. Practice Mailing Address (if different from Practice Physical Address)

1. City/Town;

2. State; and

3. Zip Code
