Section 144-220-G-L-2404 - Records of surveys of patients and human research subjects after source implant and removal

A licensee shall maintain a record of the surveys required by G.404 and G.604 for three years. Each record must include the date and results of the survey, the survey instrument used, and the name of the individual who made the survey.
