Section 144-118-3.D - Issuance of License

Each license issued by the Department shall specify:

3.D.1. The name and/or the address of the facility; 3. D. 2. The name of the administrator;

3.D.3. Whether the facility is ICF/MR Nursing or Group;

3.D.4. The maximum allowable number of clients;

3.D.5. Any waivers that have been granted; and

3.D.6. An expiration date.
