Section 143-23-1.10 - Course Instructor Application Form

An application form is to be filled out by each candidate wishing certification as a CTL Safety Instructor; documentation of prerequisites to accompany application. The following information is requested:

A. Name and legal address of applicant.

B. Date of birth of applicant.

C. Current CTL permit number.

D. Explanation of experience in Instruction/Teaching.

E. Course curriculum to be used.

F. Signature of applicant attesting all information to be true and correct.

FEE SCHEDULE:

Initial application		Renewal application
1985	$12.00	$7.00
1986	$13.00	$8.00
1987	$14.00	$9.00

These prices should remain the same unless legislative action is taken.
