For licensed, on-going beneficial use activities, the licensee shall submit, for review and approval, an annual report to the Department using the form provided by the Department. This report must contain a summary of activity during the past year, including the quantity of secondary material used by the generator or an affiliate, distributed and received for beneficial use, the sources and types of the secondary material received, and the results of any required testing or on-going characterization. For reports that include fuel substitution, any new source(s) of substitute fuel must be identified. Where required by license condition, the licensee is required to include in the annual report the locations of the beneficial use activity for the past year.
