Section 643-7-2 - License RenewalA speech-language pathologist, audiologist or hearing aid dealer and fitter may renew a license by:
1. Submitting a renewal application;2. Attesting to completion of continuing education as required by Chapter 8 of the board's rules. Timely completion of continuing education is a condition of renewal;3. Remitting the license fee required by Chapter 10, Section 5(38) of the Rules of the Office of Professional and Occupational Regulation, entitled "Establishment of License Fees;"4. For licenses renewed up to 90 days after the date of expiration, remitting the late fee required by Chapter 11, Section 2(1) of the Rules of the Office of Professional and Occupational Regulation, entitled "Late Renewals;" and5. Providing such other information as the board may require.
