Section 371-7-2 - APPLICATION PROCESSA. The applicant for a temporary license shall apply on forms provided by the Board, shall provide such information as is required by such forms, shall submit such letters of reference or recommendation as may be required, and shall present himself/herself to the Board for orientation.B. The application shall contain a notarized written statement by the owner or representative of the governing body of the facility certifying that its position of Administrator has unexpectedly become vacant; that it plans to hire the applicant, if issued a temporary license; and that it will retain a Licensed Administrator Consultant during the period in which the applicant renders services to it under a temporary license.
