The complaint shall be submitted in writing to the Board in original and four (4) copies shall be signed under oath before a Notary Public and shall contain a sworn declaration or affirmation by the person signing, under penalty of perjury, that its contents are true and correct to the best of the subscriber's knowledge and belief. The complaining party shall be expected to support the complaint.
