Section V-7525 - [Effective 1/20/2025] DataA. Admission Data 1. The provider shall maintain accurate records on all new admissions, to include the following data fields: a. demographics of youth admitted, aggregated by: v. parish of residence; andvi. geographical zone determined by provider to include zip code, local law enforcement zones);b. legal status of youth, aggregated by: i. custody status of the youth; andii. adjudication status; (a) pre-adjudicated-status/delinquent; and(b) post-adjudicated-status/delinquent;c. offenses of youth admitted, aggregated by: d. youth participation in Families in Need of Services (FINS) program: i. dates of participation in FINS (formal or informal); and/orii. referrals to FINS (formal or informal).B. Operational Data 1. The provider shall maintain accurate records of operational events that include the following data fields: a. youth released, aggregated by: b. average daily population of youth in the facility; andc. average length of stay of youth in the facility.C. Detention Screening Data 1. If a provider conducts a Risk Assessment Instrument (RAI) on new admissions, it shall maintain an accurate record of the following data fields: a. demographics of youth screened, aggregated by: v. parish of residence; andvi. geographical zone determined by provider to include zip code, local law enforcement zones;b. offense of youth screened: i. specific charge(s); andc. screen data: iii. screening outcomes: release/alternative to detention/secure detention;d. outcome data: i. successful/unsuccessful; andii. recidivism/failure to appear (FTA).La.
