Section XV-919 - CorrespondenceA. All correspondence and or notifications to the department must come from the municipality, parish, or fire protection district, not the individual recipient. In all correspondence or notifications, please include the applicant or recipient's name, last four digits of social security number, and the municipality's contact information.
