Section III-1913 - Additional Identification Requirements for MinorsA. Minor applicants applying for a first-time special identification card or a duplicate special identification card, shall be accompanied by a parent, a custodial parent if the parents are divorced or legally separated, or legal guardian if a person other than one of the parents has legal custody of the minor. The signature of a parent, a custodial parent if the parents are divorced or legally separated, or legal guardian if a person other than one of the parents has legal custody of the minor shall be required for the issuance of a special identification card. Proper identification shall be presented by both the minor and parent or guardian. The department may require additional documentation or information in order to verify the identity of the minor and parent or legal guardian.La. Admin. Code tit. 55, § III-1913
