Section XXVIII-309 - Renewal of Certificate of Registration [formerly paragraph 28:022]A. [Formerly paragraph 28:022-1] Each registrant shall file applications for renewal of certificate of registration annually on forms provided by the department. The renewal application shall be forwarded to the mailing address of the registrant as listed on the last application for registration submitted to the department.B. [Formerly paragraph 28:022-2] The following fees shall accompany each application for registration renewal. Registrant | Renewal Fee |
Owner of Facility | $ 500 |
Manager of Facility | $ 150 |
Operator | $ 60 |
Training Facility or Person | $1,000 |
1. Make check or money orders payable to the Department of Health and Hospitals.C. [Formerly paragraph 28:022-3] Provided that a registrant files a required application with the department in proper form not less than 30 days prior to the expiration date stated on the certificate of registration, the certificate shall not expire pending final action on the application by the department.La.
