Section XXI-2105 - Request for Services Registry [Formerly Section 2107]A. The Department of Health is responsible for the Request for Services Registry, hereafter referred to as "the registry", for the ADHC Waiver. An individual who wishes to have his or her name placed on the registry shall contact a toll free telephone number, which shall be maintained by LDH.B. Individuals who desire their name to be placed on the ADHC waiver registry shall be screened to determine whether they: 1. meet nursing facility level of care; and2. are members of the target population as identified in the federally-approved waiver document.C. Only individuals who pass the screening in §2105.B shall be added to the registry.La. Admin. Code tit. 50, § XXI-2105
