Section XXI-13915 - Transportation for Day Habilitation and Supported Employment ModelsA. Transportation provided for the beneficiary to the site of the day habilitation or supported employment model, or between the day habilitation and supported employment model site (if the beneficiary receives services in more than one place) is reimbursable when day habilitation or supported employment model has been provided. Reimbursement may be made for a one-way trip. There is a maximum fee per day that can be charged for transportation regardless of the number of trips per day. 1. Transportation is included in the group supported employment service rate when traveling between job sites.2. Transportation is a separate billable service if criteria is met. One rate covers regular transportation, and a separate rate covers wheelchair transportation.3. Transportation may be provided to and/or from the beneficiary's residence or a location agreed upon by the beneficiary or authorized representative to the onsite location or community location and a separate return trip.B. Licensing Requirements. Providers must be licensed by the Department of Health as a home and communitybased services provider and meet the module specific requirements for the service being provided. The provider must have insurance coverage on any vehicles used in transporting a beneficiary that meets current home and community-based services providers licensing standards.La. Admin.
