Section XV-8721 - Cochlear Implant Team

A. Team Minimum Requirements. The implant team shall be composed of the following members at a minimum:

1. physician/otologist;

2. audiologist;

3. speech/language pathologist;

4. psychiatrist; and

5. educator of the deaf with experience in oral/auditory instruction.
