Section V-8105 - Consent FormsA. When an individual seeks immunization services, an immunization record is completed. The patient, parent or guardian, as appropriate, is required to execute an informed consent form. Each time the patient returns to the health unit for an additional immunization the patient, parent or guardian, as appropriate, is required to execute an informed consent form. The signed portion of the consent form is retained and filed by the health unit; the remainder of the informed consent form is returned to the patient for reference. A public health nurse shall review the consent form with the patient, parent or guardian, as appropriate, to discuss the illness, the risks, contraindications, and side effects of the vaccine and to answer any questions.La.
