Section V-8025 - Renewal of CertificationA. Applications for renewals of certification will be accepted by the department from October 15 through December 1 of each year and shall be submitted at least 30 calendar days prior to the expiration date of the current certificate on forms provided by the department. The appropriate application fee must accompany the application in accordance with §8027La. Admin. Code tit. 48, § V-8025
