Section XLV-6025 - Obligations and ResponsibilitiesA. It shall be the mutual obligation of a genetic counselor, who has a CPA with a collaborating physician, and collaborating physician to: 1. within five days, report directly to the board, in writing, of: a. the termination of the collaborative practice agreement between a collaborating physician and genetic counselor; andb. the retirement or withdrawal from active practice by the collaborating physician or genetic counselor;2. comply with reasonable requests by the board for personal appearances, information and documentation required by this Part relative to the functions, activities, and performance of the genetic counselor;3. insure that each individual to whom the genetic counselor provides patient services is expressly advised and understands that the genetic counselor is not a physician; and4. insure that, with respect to each patient, all activities, functions and services of the genetic counselor are immediately and properly documented in written or electronic form.La.
