Section XL-133 - Report of Address Changes

A. Every licensee shall report any change in office address, residence address, office phone, residence phone, and/or email address to the board, on the board approved change-of-information form posted on the boards website, within 15 days of such change. The board shall acknowledge any change, in writing, and shall update all records accordingly.
