Section LXVI-1111 - Application for Initial Licensure for Temporary Work Permit by ExaminationA. Requests for application for initial licensure and for temporary work permit by examination forms shall be requested and submitted to the state board.B. Pursuant to R.S. 37:3210(C), upon payment a temporary 90-day work permit shall be issued one time only and for the time listed on the temporary work permit.La. Admin. Code tit. 46, § LXVI-1111
