Section LX-2709 - Notification of ChangeA. Licensed marriage and family therapists, provisional licensed marriage and family therapists, and LMFT-approved supervisors/supervisors-in-training shall notify the Licensed Professional Counselors Board of Examiners in writing of any and all changes in name, address, and phone number within 30 days. Failure to do so will result in a fine as set forth in §901 CB. Every licensed or provisionally licensed marriage and family therapist shall immediately notify in writing the Licensed Professional Counselors Board of Examiners of any and all status changes with the justice system, including notification of arrest, charges, convictions. Failure to comply with this rule within 30 days of change will result in disciplinary action.La. Admin. Code tit. 46, § LX-2709
