Section XI-4105 - Incident ReportingA. Each operator that experiences an incident that is required to be reported under this Chapter shall, as soon as practicable but not later than 30 days after discovery of the incident, prepare and file an incident report on the form and in accordance with procedures established therefore by the commissioner and to the party he specifies.La.
