Section XVII-505 - ProposalA. The agency must submit a proposal to the Community Services Block Grant Section in the format developed for that purpose. The proposal must include: 1. a description of the organization and its purpose;2. a justification of the need for the program, including the identity of the method used to survey the need;3. a complete description of the services to be provided and the method of assuring the services are provided to the most needy in the area, the dates the program will begin and end, and the number of persons to be served;4. an identification of the staff required to provide the services and a brief job description of each;5. a complete line item budget for the funds required to operate the program; and6. an identification of the contact person.B. The proposal will be reviewed by the CSBG Section, and recommendations made to the Secretary of Employment and Training or designee. The decision to fund will be made by the Secretary of Employment and Training or designee, and written notification will be made.C. A subgrant will be developed from the proposal, and returned to the agency for signature. The subgrant must be signed by the authorized representative of the agency and the Secretary of Employment and Training or designee prior to the beginning of any activity, unless written authorization has been received to operate programs prior to that date.La.
