Section I-4915 - Schedule of Maximum AllowancesA. This reimbursement schedule indicates maximum allowances which constitute payment in full for the services. No additional charge to the claimant is allowed. No charge shall be submitted to the carrier/self-insured employer before the service has been performed. All procedures require prior authorization. *When appropriate, a pro rata billing of this amount should be made at the rate of $1.3333 per minute. (i.e., 15 minutes should be billed as $20.00; 20 minutes as $26.67; 30 minutes as $40.00; 40 minutes as $53.33, etc.)
**IRS Standard Mileage Rate in effect at the time services are rendered.
