Section XV-492 - Reports of Leaking or Contamination from Sealed SourcesA. The licensee or registrant shall file a report within five days with the Office of Environmental Compliance using the procedures provided in LAC 33:I.3925.B and C if the test for leakage or contamination required in accordance with LAC 33:XV.426 indicates a sealed source is leaking or a source of contamination. The report shall include the equipment involved, its model number and serial number if assigned, the estimated activity of the source, the test results, the date of the test, and the corrective action taken.La.
