Upon determination by the department that a laboratory meets all requirements necessary for certification, the laboratory management shall receive a certificate containing a unique certification number. Each certificate shall include the scope of accreditation granted to the laboratory. Each certificate shall remain valid for 12 months from the date granted to the laboratory unless revoked as specified in 28-15-35f.
