Section 100-10a-1 - Applications(a) Each application for an exempt license shall be submitted upon a form furnished by the board. This form shall contain the following in plain, legible writing: (1) The applicant's name in full; (2) the applicant's post office box address if applicable; (3) the applicant's residence address; (4) the applicant's Kansas license number; (5) a statement that the applicant does not hold oneself out to the public as being professionally engaged in the practice of the healing arts for which the applicant holds a license; (6) a statement that the applicant is no longer regularly engaged in the state of Kansas in the branch of the healing arts for which the applicant holds a license; (7) a statement describing the professional activities relating to the healing arts in which the applicant intends to engage if issued an exempt license; (8) a statement acknowledging that if the applicant is issued an exempt license, the applicant shall be subject to all provisions of the healing arts act except for complying with the requirements of continuing education; and (9) a statement acknowledging that if the applicant is issued an exempt license, the following shall apply: (A) The applicant will not be a health care provider, as defined by K.S.A. 40-3401 and amendments thereto. (B) The applicant will not be required to maintain professional liability insurance in accordance with K.S.A. 40-3401et seq.(C) Any services rendered by the applicant, while the holder of an exempt license, will not be insured or covered by the health care stabilization fund.
