Rule 875-150.8 - Workers' compensation insurance cancellation notifications

(1)Insurance company coverage. The division shall be notified by the insurance company carrying the contractor's workers' compensation insurance at the time of cancellation. The notice shall contain:

a. The name of the insurance carrier;

b. The name of the insured contractor; and

c. The date the workers' compensation coverage cancellation is effective.

(2)Self-insured contractors. The contractor shall notify the division ten days prior to any cessation in self-insurance.

(3)Noninsured contractors. The contractor shall notify the division whenever the required notice is not posted or in any change in insurance status.
