Rule 283-28.5 - Award cancellation

(1) The teacher must notify the commission within 30 days following termination or change of employment in an eligible teaching field or an eligible school or agency.

(2) The teacher is responsible for notifying the commission immediately of a change in contact information including, but not limited to, name, telephone number and e-mail address.
