Section 868 IAC 1.1-15-12 - Renewal; form required

Sec. 12.

An application for renewal of a license must contain a sworn statement signed by the licensed psychologist who has received a health service provider endorsement under IC 25-33-1-5.1(c) attesting that the psychologist has fulfilled the continuing education requirements under IC 25-33-2.
