Section 868 IAC 1.1-12-4 - Duplicate license or renewal cards

Sec. 4.

(a) Upon receipt of satisfactory evidence that a renewal card has been lost, stolen, mutilated, or destroyed, the board may issue a duplicate renewal card upon such conditions as the board may prescribe and payment of the required fee.

(b) A duplicate wall license to practice psychology in Indiana will be issued upon request and payment of the required fee.
