Section 864 IAC 1.1-9-1 - Notification of name and address change

Sec. 1.

(a) It shall be the responsibility of each registrant to keep the board advised of the registrant's latest address within thirty (30) days of the address change.
(b) It shall be the responsibility of each registrant to keep the board advised of the registrant's full name within thirty (30) days of the name change.
