Section 810 IAC 2-1-4 - Registration of licensed persons on the registry

Sec. 4.

(a) The registration to be completed by an approved person licensed to provide health care services for inclusion on the registry shall be made upon an electronic form published online by the agency.
(b) All information in the registration shall be submitted under oath or affirmation, subject to the penalties of perjury.
(c) The registration form submitted to the agency shall be complete.
