Section 760 IAC 5-5-1 - Annual reporting

Sec. 1.

A pharmacy benefit manager must file annually by June 1 a report containing data from the immediately preceding calendar year on a form provided by the commissioner. The report must contain the information set forth in IC 27-1-24.5-21(a).
