Section 455 IAC 2-8-3 - Transfer of individual's records upon change of provider

Sec. 3.

If an individual changes providers for a home and community based service, the original provider shall transfer copies of all records related to the individual to the new provider:

(1) within five (5) calendar days; and
(2) in compliance with HIPAA regulations.
