Section 455 IAC 2-17-5 - Availability of providers

Sec. 5.

A provider of CMS shall provide the:

(1) individual; or

(2) individual's legal representative upon request;

with a current list of providers approved under this article, including a complete description of services offered by each provider from a generated pick list.
